Student Intake
The student intake process involves a meeting with the student and Disability Advisor to discuss student accommodations and services. During the intake process the Disability Advisor will discuss/assess:
- The student's course and/or program interests and goals.
- The disability accommodations requested by the student.
- The student's need for disability accommodations. Documentation of a disability must support the need for accommodation and be provided in writing by a relevant specialist.
- Accommodation Needs.
- Disability Documentation.
- Technical and Adaptive Equipment Requirements.
- Exam Accommodation Requirements.
- Instructor Notification of Disability form.
- Disability Funding Options.
- Alternate Format Requirements.
- Student confidentiality and Consent form.
- Student responsibilities and Statement of Understanding form.
- How to make an appointment with the Learning Specialist.
- How to make a Referral to Other Support Services as needed.
