Log in to myKwantlen, and select "View & Print T2202A Information" from the Student Records menu in the Online Self-Service menu.

KSA Extended Health and Dental Plan
Created by a student referendum, the KSA Extended Health and Dental Plan provides mandatory extended health and dental insurance for students at Kwantlen Polytechnic University. Enrolment in these plans is a requirement of registration at the University for all students with a course load of 60% (9 credits) or more. Details of the plan can be seen at www.gallivan.ca
There is a provision for opting out of either or both plans if a student is able to provide proof of comparable existing coverage. However, you can combine your existing coverage with the KSA Extended Health and Dental plan to increase your overall coverage (not all plans cover you 100%). Here are a few unique benefits of the KSA Extended Health and Dental Plan.
Students who choose to opt out, must do so no later than 30 days after the start of the semester. Opting out can be done online using myKwantlen, or by completing the Student Benefits Waiver Form.
For more information please contact the KSA Extended Health and Dental Coordinator at 604-599-2431 or benefits@kusa.ca

Math and English testing
If you want to enrol in an English or Math course for the first time at Kwantlen, you may have to write a placement test. Ideally, tests should be written at least four (4) weeks before you register. You may sign up at any Enrolment and Registrar Services office to write the Kwantlen Mathematics Placement Test (MPT) and the English Placement Test (EPT). For information on minimum scores, see the prerequisites listed in the Course section of this guide or the calendar. Applications to write the Language Proficiency Index (LPI) test are available at all Enrolment and Registrar Services offices. Students who have written the LPI test must notify the Enrolment and Registrar Services office directly so LPI results can be recorded on their Kwantlen record.
Guided study
Guided study refers to the offering of a course using a mode of instruction other than classroom instruction or distributed learning. In general, this will primarily involve self-study by the student with periodic guidance from an instructor. Guided study is not to be construed as instruction by correspondence or distance education.
See policy C.16 that can be viewed at the Library or online at www.kwantlen.ca/policies for further details.
Confirmation of Enrolment/Enrolment Verification
If you require confirmation of enrolment (enrolment verification) for a semester, you can come to any Enrolment and Registrar Services office on any campus to obtain one. They can be done right at the counter and only take a moment to print out.
If you are not able to come to a campus you may e-mail admissions@kwantlen.ca (remember to use your Kwantlen e-mail address). Include your name, student number, and the semester you want the confirmation for. Also, please tell us which campus you will pick it up at, or if you would like it mailed. In order to ensure your confidentiality, we will mail it to the address we have on file. NOTE: Enrolment verifications are not a substitute for a transcript; they do not include grades, credentials, etc.
